The Health Questionnaire is to be completed and brought with you on your first visit. We'll review the information and use this to help ensure you receive the best possible care during your initial consultation.
Use this CareCredit form to make arrangements to pay over time with low monthly payments. Although our office has no direct affiliation with this plan provider, we are happy to assist you in whatever way we can to secure financing for your dental work. Please print and complete the form, returning it to the office. We'll work with you to gain approval for the credit program quickly—while you wait.
